**Important: Educational Benefits form**
Warren Consolidated Schools is required to collect information from families to secure additional federal and state funding. The Education Benefits Form collects information needed to ensure your student’s school receives state and federal funding for education programs. We need all families who have not completed the Educational Benefits form to please take a moment to complete the form. Without this information, your student’s school potentially could miss out on important funding for education programs that our students need.
- There are three methods to complete the form
- Online: https://warren.familyportal.cloud/ and click "Apply for Benefits"
- Forms will be available at registration to complete in person.
- The form is available to download below in the Registration Information section. The form is pink. If you choose this method please print, complete the form, and return it to Warren Mott High School or the Administration Building.

Registration Directions
- Student Verification Form must be completed through the Parent Portal on PowerSchool
- If you have a Parental Portal Account, use the “Parents with Portal Accounts” direction sheet to help you complete the student verification form.
- If you do not have a Parent Portal Account, use the “Creating a Parent Portal Login” direction sheet to create a Parent Portal Account. Once your Parent Portal Account is created, use the “Parents with Portal Accounts” to help you complete the student verification form.
- Please review all the information for each of your students.
- Staff will be available to help complete your student’s verification forms during registration/picture day.

Registration Information
1. Registration/Picture Day Schedule
- August 20:
- 12th grade, 8:30-11:00 am
- 10th grade, 12:00-2:30 pm
- August 21:
- 11th grade, 8:30-11:00 am
- 9th grade, 12:00-2:30 pm
2. Freshman Orientation
- Freshman Orientation is Thursday, August 22, 2023, from 10:00 AM to 12:00 PM in the cafe at Warren Mott High School.
- Your students will receive their schedule and locker information at orientation.
- Your student is encouraged not to bring backpacks, purses, or other like items, as the day is quite active. Please make sure that your student wears comfortable clothing that does not restrict movement.
- To ease the anxiety of the first day of school
- To engage in activities that demonstrate how to be successful in high school.
- To gain a general familiarity with the building, specifically classroom location.
3. Warren Mott Dress Code
Students who violate the dress code will be sent to their House Office to change into school issued clothes. Students must return the school issued clothing by the next day or a fine will apply. ($25/pant and $10/shirt).
Students with chronic dress code issues may be subject to disciplinary action under the terms of the Code of Conduct.
- WMHS 2024-2025 IDs are to be worn and remain visible while on school property. Should IDs be altered in anyway (Sharpie, stickers, etc.), a new ID must be purchased.
- Clothing must be worn in an appropriate manner for school.
- Clothing, including footwear and accessories, that may damage furniture or property, or cause injury to people, cannot be worn.
- No clothing may be worn that causes a distraction or disruption of educational programs, including references to inappropriate language or pictures, cigarettes, vapes, drugs, alcohol or illegal activity.
- All coats and backpacks must be stored in your locker throughout the instructional day, including lunch. Administrative discretion may be used.
- Clothing that reveals cleavage, midriff, backs, and undergarments is NOT permitted.
- Bare shoulders and tank tops are NOT permitted.
